Thank you for your question.

There are some mandatory offences where people are automatically placed on the barred list but the two areas that you raise:

1. A medication error
2. Mistake in weighing people

would most definitely not lead to an automatic barring.

If you were to be reported to the DBS by an employer or some other organisation you would be informed that a complaint had been made about you and you would be invited to respond to whatever the complaint was.

No one can be barred without going through the appropriate process.
