There is not a great deal of information in your email but I am going to assume that you are already in contact with your local social services department.

If you are not you should contact them.

They have the responsibility to work with and support people who are in need of care and who will need assistance with finding that care and with the payment for that care.

The social services department will assess you to identify the needs that you have and then work with you to decide how those needs can be met.
